Charisse Hunter is the director of care management at Aledade. She is the thought leader for strategies to support high-risk and high-cost patients. This includes programs such as chronic care management, behavioral health care management, end-of-life care, and medication management. Hunter is a registered nurse with a background in medical intensive care nursing, primarily with MedStar Health’s Georgetown University Hospital. Following several years of hospital-based nursing, Hunter pursued her master’s in health care administration with a focus on executive management and quality improvement. She has since transitioned her nursing career to focus on outcomes management and population health, particularly in the health plan and accountable care space. Hunter most recently worked at Evolent Health where she was a nursing leader in both clinical program design and optimization and care management operations for a vulnerable Medicare special needs population.
